Health Information
Adult Primary Care accepts most insurance plans as well as Medicare. We submit claims to insurance companies on your behalf. If your insurance plan requires a co-payment for an office visit, this is expected at the time of the visit. If you cannot pay the balance, please let us know. We would be happy to make payment arrangements with you.
Be sure to bring your insurance card with you to your visit. It is your responsibility to notify us of any changes to your insurance coverage.
In the event that you have coverage but cannot provide documentation, payment is due at the time of your visit. After you provide us with your information, we will then submit a health insurance claim form to your insurance company indicating that you have paid for your visit and refund any benefits paid by your insurance company to you.
